Most healthcare clinics are still marketing the way they did three years ago — and losing patients to competitors they have never heard of.
The patient journey has changed, and it is changing faster than most clinics realise. In a recent webinar, 2Stallions co-founder Razy Shah and marketing manager Natalia Lukito walked healthcare professionals from Singapore and Malaysia through what is actually working today. Here is a full summary.
How the Patient Journey Has Changed
Before AI became mainstream, the patient journey was predictable. A patient felt a symptom, searched Google, browsed a few clinic websites, and booked an appointment. The marketing playbook was simple: rank on the first page of Google and the patients would come.
That linear path no longer exists.
Today, patients move across multiple channels at different stages of their journey:
- Researching symptoms – Patients now frequently turn to ChatGPT and other large language models (LLMs) first, asking AI directly: “What could this pain in my chest mean?” rather than typing it into Google.
- Searching for a provider – When it comes time to find a clinic, patients lean more heavily on Google. They want to browse individual websites, read doctor profiles, and assess credibility before making a decision.
- Validating their choice – Reddit threads, Quora answers, YouTube explainers, and Google reviews all play a role. Reddit recommendations for local specialists are increasingly common in search results. YouTube has become a key channel for patients who prefer watching a doctor explain a procedure over reading a long article.
The takeaway: a marketing strategy that only targets Google is now leaving significant visibility on the table.
The New Search Landscape : Google, AI Overviews, and ChatGPT
Let us address the most common misconception first. Google is not dead. Google processes approximately 13.7 billion searches per day, compared to ChatGPT’s 2.5 billion daily prompts. Google remains by far the dominant search channel and, crucially, it is still the channel most likely to drive actual website traffic. People searching on Google typically have commercial intent — they want to find and evaluate a specific provider.
ChatGPT users, by contrast, tend to seek informational answers and are less likely to click through to individual websites.
What is disrupting traditional SEO significantly is Google’s AI Overviews.
When a patient searches “best cardiologist in Singapore,” they no longer necessarily see organic listings first. A large AI-generated summary now appears at the very top of the results page — before the map pack, before any organic links. The impact is significant: AI Overviews are known to reduce clicks to individual websites by up to 58%. They appear on approximately 60–65% of searches today.
Patients read the AI summary, pick a few names mentioned in it, and search for those directly — without ever scrolling down to your organic listing.
This is why ranking on page one is no longer enough. Clinics must now optimise not just for Google rankings, but for being cited by AI.
SEO vs. GEO: What’s the Difference for Healthcare Marketing?
You have likely heard the terms flying around — SEO, GEO, AEO, AI SEO. Here is a simple way to think about them:
| SEO | GEO / AEO | |
| Goal | Rank on Google’s first page | Be cited or recommended by AI |
| Channels | Google, Bing | ChatGPT, Google AI Overviews, Gemini, Claude |
| Metric | Rankings and organic traffic | Citations and AI mentions |
The key distinction: in traditional SEO, you aim to rank. In GEO (Generative Engine Optimisation), you aim to be cited.
Importantly, these are not competing strategies – they work together. The SEO foundations you build (quality content, backlinks, technical site health) are the same foundations that improve your chances of being cited by AI. You cannot skip SEO and jump straight to GEO. A complete organic strategy in 2026 and beyond requires doing both.
What Google and AI Look for in Healthcare Websites
The SEO Ranking Factors That Matter Most
- Quality content and engagement signals – Google measures how long users spend on a page, how far they scroll, and whether they leave immediately. Pages with strong engagement get rewarded; pages that users abandon within seconds get downranked over time.
- Backlinks and domain authority – Links from reputable websites still matter, especially from healthcare-specific sources: medical associations, government health portals such as MOH, health directories, and credible media mentions.
- Technical SEO foundations – A slow, error-ridden website undermines all other efforts. Broken links, missing images, and poor loading speeds send negative signals to Google and reduce the likelihood of AI crawling and indexing your content properly.
- Brand signals – How often your clinic is mentioned across the digital ecosystem including Reddit, Instagram, blogs, news articles, and patient reviews, contributes to how credible Google perceives your brand.
- User experience – Website speed, accessibility, and ease of navigation all contribute to ranking. A fast, well-structured website is no longer optional.
The Additional Signals AI Looks For (GEO)
On top of the SEO foundations, AI engines place extra weight on:
- Doctor credentials and experience – If a clinic’s website clearly states that its doctors have 10+ years of experience or cover distinct sub-specialisations, it reads as more credible to AI.
- Depth of specialisation – A cardiology clinic with multiple doctors covering different areas of cardiology appears more authoritative than a single-doctor practice with minimal information, even if the single doctor is equally skilled.
- Google Reviews – AI actively looks at the number and quality of Google reviews when making recommendations. Review volume and average rating directly influence which clinics get cited.
- Online mentions and Reddit/Quora recommendations – The more your clinic is referenced across trustworthy platforms, including user recommendations on Reddit and Quora, which AI actively references – the more AI is inclined to recommend it.
- Schema markup – Implementing structured data using MedicalBusiness, Procedure, and Condition schema helps AI correctly identify your clinic, doctors, and services. This is a technical but important GEO signal.
Building Your SEO Foundation for Healthcare
Keyword Research
Understand exactly what terms your patients are searching for, from broad terms like “cardiologist Singapore” to specific ones like “cardiac stress test Singapore.” Build your content and service pages around these terms.
Condition and Treatment Pages
Every key condition or treatment your clinic addresses should have its own dedicated, comprehensive page. If patients search for it, you need a page for it. These pages must be well-written, SEO-optimised, and detailed enough to demonstrate genuine clinical expertise.
Local SEO
For most healthcare businesses – GPs, physiotherapists, dental clinics, and similar to local SEO is critical. Patients search for clinics near them. This means:
- Optimising your Google Business Profile with accurate categories, photos of your team and clinic, booking links, and complete service listings.
- Using location-specific keywords in your website metadata where relevant, based on actual search volume data. For example, “Physiotherapist in Singapore” rather than just “Physiotherapist.”
- Actively collecting Google reviews. Your local pack ranking and AI visibility both depend heavily on review volume and recency.
Content Marketing
Content and SEO are inseparable. A strong content library covering every condition, treatment, and common patient question in your niche signals to both Google and AI that your clinic is an authority in its field.
One important note: AI-generated content without significant human editing tends to underperform. Google and AI platforms can detect lower-quality, purely generated content. The best approach is to use AI as a drafting tool, then have the content reviewed, medically verified, and where possible, attributed to a named doctor as the author. FAQ sections and conversational tones are particularly effective for GEO.
Paid Advertising: What Works Best for Healthcare Marketing
Google Search Ads
Still the highest-performing paid channel for healthcare lead generation. The key advantage: you can target keywords with high commercial intent. Someone searching “chiropractor near me” or “aesthetic clinic Orchard” is actively looking to book. Google Search Ads let you appear at the top of results immediately, without waiting for organic rankings to build.
Search Ads generate the best-quality leads. Display and Performance Max are better suited for remarketing — showing your brand again to people who have already visited your website.
Meta Ads (Facebook and Instagram)
Meta Ads are less effective at direct lead generation for healthcare compared to Google, but they shine in two scenarios:
- Retargeting – The Meta Pixel tracks website visitors and serves ads to them as they browse Instagram or Facebook, keeping your clinic top of mind for patients who visited but did not enquire.
- Demographic targeting – For specialties with specific target audiences, aesthetics targeting older females, fertility targeting couples, and paediatrics targeting parents, Meta’s demographic targeting can be highly effective.
- Click-to-WhatsApp Ads are a particularly strong feature for healthcare in Singapore and Malaysia. Patients see an ad, click, and connect directly via WhatsApp, a frictionless path to enquiry that works especially well in markets where patients want to ask questions before committing to a visit.
Organic Social Media
Organic social is generally not a primary lead-generating channel for healthcare. However, patients who are deciding between clinics often check social media as a final credibility check. A professional, engaging presence showing doctors, helpful content, and patient testimonials can tip the decision. If resources are limited, prioritise SEO and your website first, then ads, then organic social.
Healthcare Marketing Compliance: What Clinics in Singapore and Malaysia Need to Know
Healthcare marketing is regulated. In Singapore, this falls under the Healthcare Services Act (HCSA); in Malaysia, the Medicine Advertisements Board (MAB) and Malaysian Medical Council (MMC) guidelines apply. All content — website, ads, and social media — must be reviewed for compliance before going live.
SEO vs. Paid Ads: Which Should Healthcare Clinics Choose?
| SEO | Ads | |
| Time to results | Slower (3–6 months) | Fast — results from day one |
| Cost model | Agency fees; traffic is free once ranking | Pay per click — ongoing spend required |
| Long-term ROI | Higher — results compound over time | Stops when budget stops |
| Best for | Sustainable long-term growth | Generating leads quickly |
The ideal approach for most competitive medical brands is to do both. SEO builds your long-term organic presence; ads fill the gap while SEO builds momentum, and continue to capture high-intent patients who prefer clicking on paid results.
If you can only choose one and need patients quickly: start with Google Ads. If you can wait and want sustainable growth: start with SEO.
The Clinics That Get Found Have Already Adapted
The patient journey is more fragmented than ever — and the clinics that grow from here will be the ones that adapt to where patients are actually searching, not where they used to search.
The good news is that the fundamentals still work. SEO, paid ads, and content marketing are not obsolete — they just need to be updated for a world where AI Overviews, ChatGPT, and Google Maps all play a role in how patients find and choose a clinic.
Start with the right foundations, keep your content clinician-led, and make sure your digital presence extends beyond your website. That is what separates the clinics that get found from the ones that don’t.
